Our goal is to connect you with the best care and resources possible. Information Sharing Your information may be shared within our trusted Wellness Network to coordinate care and connect you with services that best meet your needs. All information is shared securely and only with professionals directly involved in your care. Family Navigator Meetings Meetings are specifically to connect with our Family Navigators for support, guidance, and resource navigation. To best ensure support, meetings must be booked at least 24 hours in advance. Emergencies If you are in crisis or experiencing an emergency, please call 911 immediately.
